Home Care
Home care provides support for people in their own homes, helping with daily tasks, personal care, medication, mobility, and companionship. It can be short-term, long-term, or temporary, depending on individual needs.
Home care is suitable for Adults over the age of 18 who need support at home due to, illness, disability, or recovery after surgery. We provide care for adults, older people, and those with conditions such as dementia, Parkinson’s, or long-term health needs.
We start by a visit with a Health Care Assessor, who will assess your needs, preferences, and routines. Based on this, we create a personalised care plan that can be adjusted as needs change. Care can be provided for a few hours a day, overnight, live-in, or on a flexible schedule. We will be with you every step of the way.
Our carers can support daily healthcare routines, such as medication reminders, monitoring wellbeing, and assisting with mobility or exercises recommended by healthcare professionals. We work closely with GPs, nurses, and Occupational therapists, District nurses, Parkinsons nurses, hospital discharge teams, frailty teams, falls teams and social workers, to keep you safe.
Respite care is short-term support provided to give family carers a break. It can range from a few hours to several days and helps ensure the individual continues to receive safe, compassionate care at home. We will plan and work with you so the respite supports you and your family, to have peace of mind. It is important to us that you remain the daughter or son, mother or father.
Palliative care provides comfort and symptom management at any stage of a serious illness. End of life care is focused on the final stages, supporting comfort, dignity, and peace. Both types of care are delivered with compassion and in partnership with healthcare professionals.
All our carers are carefully vetted, trained, and experienced. We match carers to the individual’s needs and personality to provide consistent, reliable, and compassionate support.We try to keep your team small, and we do introduce the carers to you via shadowing to alleviate confusion and stress.
Yes. We understand that care needs can arise suddenly, and we aim to provide flexible, timely support tailored to your situation.
Care plans will establish the areas where you need support, no task is too small, we are happy to empty bins, laundry, visits to the shops for fresh products. Assisting to appointments and communicating with external professionals.
We maintain regular communication with families, providing updates, guidance, and reassurance. Families are encouraged to be involved in care planning and decisions at every stage. We can add you to our digital system for transparency, it is clear and simple to use and you can see the case notes, care plan, rota, and the risk assessments. We inform you of any changes, and we will regularly check in and always keep you up to date. We also use WhatsApp groups so you can communicate with your care team for immediate response.
The cost depends on the level and duration of care needed. We provide a clear, personalised estimate during the initial assessment, with no hidden fees. We can also advise on local funding options if appropriate.
Care plans are regularly reviewed and can be updated as the individual’s needs evolve, ensuring the right level of support is always in place.
